Photo use permission form
I, Gianni Bozzacchi, hereby grant _________________________ the non-exclusive right and permission for a one-time use of specific photographs listed below, which are also part of the body of work written by Gianni Bozzacchi, entitled Elizabeth Taylor: The Queen and I under the following terms:
a) To use, publish, in whole or in part, individually or with other photographs or images in conjunction with a review or feature story of the book Elizabeth Taylor: The Queen and I by Gianni Bozzacchi in any medium including (but not by way of limitation) illustration, promotion, advertising and trade, one time only.
b) The right of one-time use is for a period of 3 calendar months from the date of the last signature below. Any further use of this material will require a separate request.
c) The Photographer warrants that he is the sole and exclusive owner of these photographs, that they do not infringe upon any statutory copyright or upon any common-law, proprietary, or any other right of any kind; that these photographs are not in violation of any right of privacy, or otherwise contrary to law; and that he has the full power to grant this permission to the Requestor.
